Can my child get vaccinated for COVID-19?

No. More studies need to be conducted before COVID-19 vaccines are recommended for children aged 12 and younger. Currently only the Pfizer vaccine is approved for children 12 and over.

Do I need an appointment for a COVID test?

Appointments are preferred. Please call 520-350-7585 to schedule or book online.

From your Exhibit A2, two (2) buildings are shown. Is that correct or is the only one (1) 130K SF building? Also is the warehouse building part of the project? I assume there is no complete set of plans with architectural and structural drawings available at this time. When will they be available?

We are still in the early planning stages with our architect and are still contemplating whether we want one 130K SF building on the site.  Our initial thoughts included having a separate building for warehouse and facilities maintenance functions.

Our goal is to incorporate the selected GC as part of the process for development of the plans for this project as feasible.

How do I choose a primary care provider?

Patients have options in choosing a primary care provider based on personal preference, medical needs and provider expertise. At Sun Life Health, you have access to adult and pediatric primary care practitioners, behavioral health providers and dental* professionals. The health center providers are all board certified, with significant expertise and experience in their field of study. As a medical patient, you may have the option to choose between a Doctor, a Physician Assistant and a Nurse Practitioner.

How do I get a referral? How does the center ensure that the referral is obtained and report received?

The best way to obtain a referral is to schedule an appointment with your provider so that a referral can be done in a more informed and efficient manner. After consulting with your provider, a mutual decision is made to refer you for diagnostic testing or speciality care depending on your needs. Your provider will make a referral to the referral team and the referral team will work with your insurance company to ensure that the services you’ve been referred to is approved by your insurance agency and to schedule an appointment.

How do I get speciality care?

Coordinated care is when all parts of the health care system talk to each other. At Sun Life Health, we coordinate care with your specialists, hospitals where you receive care, home care teams involved in your care and other support services. It is important that when you receive outside speciality care, your records are shared with your PCP.

How is care coordinated across the health center and with outside specialists?

At Sun Life Health, care coordination begins with your Circle of Care Team, which consists of you, your provider, medical assistant, care manager, and nurse. Your team is required to understand you, understand your family dynamics, unique needs, your values and preferences, even your cultural orientation and background. Depending on your needs, other members of the team may include medical specialists, behavioral health and substance abuse providers and/or clinical pharmacist(s).

Your Circle of Care Team is actively involved in working to provide you with the best care possible by accessing different disciplines in the health center and outside. Your Circle of Care Team works closely with local hospitals, off-site specialists and community resources to ensure that we meet the needs of our patients and we provide timely and accurate follow through.

How much does the vaccine cost?

Currently there is no cost to receive the vaccine. If applicable, we will bill your insurance.

I’m having non-emergency side effects from the covid vaccine, should I report them?

Yes. Using the V-Safe After Vaccination Health Checker from the CDC.
V-safe is a smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccination. Through v-safe, you can quickly tell CDC if you have any side effects after getting the COVID-19 vaccine. Depending on your answers, someone from CDC may call to check on you and get more information. And v-safe will remind you to get your second COVID-19 vaccine dose if you need one.

If I already had COVID-19 and recovered, do I still need to get vaccinated?

Yes. CDC recommends that you get vaccinated even if you have already had COVID-19, because you can catch it more than once. While you may have some short-term antibody protection after recovering from COVID-19, we don’t know how long this protection will last.

What ages can be tested for COVID?

Tests are available for people of all ages with an appointment. Walk-In testing is available for ages 2 and up, minors need to be accompanied by a parent/guardian.

What is the cost for COVID testing?

The out-of-pocket cost for molecular PCR COVID testing is $110. We will bill insurances as applicable and patients will be responsible for any patient portion determined by their insurance.

What type of COVID test do you offer?

Our curbside test is a COVID-19 active antigen (PCR) nasal swab test.

When will I get my test results?

Results depend on laboratory timelines and are dependent on their schedules, sometimes up to 7 days.We use both Labcorp and Sonora Quest.

Which vaccine does Sun Life Health provide?

Sun Life Health administers the Moderna vaccine for ages 18 and over in Casa Grande, Maricopa, Oracle and San Manuel. Adult and Pediatric Pfizer vaccine are available in Casa Grande.
